Edited 12/4/18 – A MVS Care Coordinator is at CMC five days a week, and at the Elliot four days a week.

Easterseals NH Military Veterans Services (MVS) in partnership with Catholic Medical Center (CMC) and Elliot Health System are pleased to announce a first-of-its-kind, hospital-based Care Coordination program to serve patients and employees who are veterans, service members, and family members.

To further serve the unique needs of veterans and service members, CMC and Elliot Health System, will supplement their current Care Coordination team with an embedded Easterseals MVS Care Coordinator at each facility.

These services will include, but are not limited to resource referral, substance use disorder treatment and wrap-around care, general care coordination, housing and homelessness, employment, women’s issues, military sexual trauma, aging and senior issues, discharge planning, and care transition. In addition to services, linkages may also be available to health care, benefits, legal issues, mental health services, counseling, and emotional support resources for individuals, couples, and children.

Additionally, emergency financial assistance may be provided through Veterans Count, a fundraising program of Easterseals, that provides critical and timely financial assistance and services when no other resource is available to veterans, service members and their families, to ensure their dignity, health and overall well-being. When assistance is provided, it is done so as a hand-up, not a hand out and in a larger plan for self-sufficiency is developed.

These free, confidential, and local services will be provided to veterans of any era regardless of VA eligibility, discharge status, length of service, or active duty status.

Since Easterseals launched the MVS program in 2006, more than 11,280 veterans, service members, and their families have been served. The partnerships with CMC and Elliot Health System will broaden the impact of care coordination across the social determinates of health such as housing, employment and behavioral health.

“The members of our military and their families sacrifice so much for the freedom of all and it is our duty to support them in their time of need,” said Easterseals NH President & CEO Larry Gammon. “We are honored to partner with the dedicated teams at Catholic Medical Center and Elliot Health System to ensure our heroes get the care they deserve.”

With the MVS Care Coordination Model embedded at CMC and Elliot Health System, service members will easily connect to preventative care and community resources with a goal of reducing hospital stays and future emergency room visits.

As service member and veteran-focused healthcare facilities, CMC and Elliot Health System will each work with Easterseals MVS to provide a coordinated and integrated effort in the development, promotion, and operation of case management services in order to maximize the health and well-being of the service members and veterans receiving care at their hospitals

The MVS Care Coordinator will be on staff three days a week at CMC at 100 McGregor Street in Manchester, NH.

“Collaboration is essential in delivering on our promise of excellent care – the care our veterans deserve,” says CMC President & CEO Joseph Pepe, MD. “CMC is proud to be a leader in breaking down barriers for veterans and service members to access quality health care.  Working with our partners at Easterseals and Veterans Count ensures New Hampshire military men, women, and families have all the services they need.”

The MVS Care Coordinator will be on staff two days a week at the Elliot Hospital main campus at 1 Elliot Way in Manchester, NH.

“Elliot is proud to serve the healthcare needs of veterans and their families throughout southern New Hampshire for over 125 years and we respect and honor their service,” said Greg Baxter, MD, Chief Medical Officer. “This partnership will be very impactful to our patient-veterans as it capitalizes on the unique care coordination skills of Easterseals and the resources that will now become available as a free service embedded in the Elliot Hospital. It is a true community focused model that promises access to the right programs for service members and veterans and we all know that access to the right service is often the key to improving the health of our community.  We are thrilled to be a part of this program and thank and applaud Easterseals for their vision,” Baxter added.

Easterseals has been providing culturally competent care coordination for service members, veterans and their families for over a decade and are adept at navigating complex systems creating barrier free pathways to services while increasing the capacity of the individual to become self-sufficient. Veterans, service members, or military families who are patients or employees at the CMC or Elliot Health System, who are in need, should contact Leslie at 603.851.0281.
